Community Faculty Resources

A New Formula for Child Health (the CHPT Casebook): A project funded by Sierra Health Foundation and co-developed by the Asset-Based Community Development Institute, this helpful guide uses real-life CHPT stories to illustrate the successes and challenges of community-physician partnerships.  Includes hands-on tools developed by CHPT partners, case-studies and interviews and so much more!
