Elizabeth Sterba, MS
: Manager

Elizabeth Sterba (Liz) has served as the CHPT Manager for over seven years, helping grow strong community leaders, community-savvy physicians and a robust training program that includes all primary care residency programs as well as medical students and public health students at the University of California, Davis.  

Liz has been a child and community activist in the Sacramento area for over fifteen years. Liz's work began in her early teens through involvement in a neighborhood health initiative, and grew into service on several local, city- and county-wide efforts, throughout high school and her undergraduate work at California State University, Sacramento, where she earned a Bachelor of Arts degree in Child Development. 

After working with CHPT as a Community Faculty in south Sacramento for several years, Liz took on the role of CHPT Manager, during which time she also earned her Master of Sciences degree in Community Development at UC Davis. 

In addition to her work with CHPT, Liz serves as an appointee and Chair of the Sacramento County Children's Coalition;  Board Member for the South Sacramento Coalition for Future Leaders;  member of the Sacramento County Children's Trust Fund committee; and volunteers with Sacramento Building Healthy Communities and the Tahoe/Colonial Collaborative.


Kay Nelsen, MD
CHPT Director in Family & Community Medicine

Kay graduated from the Bowman Gray School of Medicine and completed her residency in Family Medicine at the David Grant Medical Center at
Travis Air Force Base in California.  Following her residency training, Kay completed fellowships in family medicine at UC San Francisco and the University of North Carolina at Chapel Hill, in addition to a fellowship in Obstetrics at the Santa Clara Valley Medical Center.  

Trained to provide care for the whole family, Kay believes that collaboration is key to developing and reaching health goals, a tenet she shares with the trainees she teaches as Associate Program Director.  Kay's special interests include women's health, family-centered obstetrics and pediatrics.

Tonya Fancher, MD
CHPT Director in Internal Medicine

Tonya graduated from the NYU School of Medicine and the NYU/Bellevue Primary Care Internal Medicine Program.  She then spent 4 years teaching at the Travis Air Force Base Internal Medicine Residency Program.  In 2004 she completed the UC Davis Primary Care Outcomes Research Fellowship and joined the faculty as an Associate Program Director.  She is the co-PI for our HRSA Primary Care Residency Training Program that is the first academic partnership between UC Davis and the Sacramento County Clinics and focuses on care for the medically indigent. 

At the residency level, Tonya directs the resident research program, the community engagement curriculum, and a program with the UC Davis School of Veterinary Medicine where Internal Medicine residents collaborate with veterinarians in Rwanda.  She also directs a flagship project at the UC Davis Asian American Center for Disparities Research to improve medication adherence and depression care among Hmong and Vietnamese patients.

Albina Gogo, MD
CHPT Director in Pediatrics

Bina graduated from the UC Davis School of Medicine and completed her residency in Pediatrics at UC Davis, as well.  Spending many years as a general pediatrician in community settings, Bina returned to UC Davis as a practitioner and Associate Program Director in the Residency Section, where she teaches primary care, community pediatrics and manages the UC Davis pediatric partnership with The Effort, a local community clinic in Oak Park.


Richard Pan, MD, MPH: Founding Physician

Dr. Richard Pan is on leave from his roles as Associate Professor of Clinical Pediatrics and Director of Outreach and Government Affairs for the UCD Children’s Hospital; as he is currently serving in the California State Legislature, representing the 5th Assembly District.  

Together with leaders from Sierra Health Foundation and its Community Partnerships for Healthy Children initiative, Dr. Pan founded Communities & Health Professionals Together (CHPT - formerly Communities & Physicians Together) in 1999.  CHPT is a nationally recognized program that partners resident physicians, medical and public health students with community associations to improve child health.
