Community is Key...

...in all that Communities & Health Professionals Together does. The community provides a rich, non-traditional classroom outside the walls of the hospital and clinics necessary for residents to get the best training possible. The partnership formed by community members and UC Davis ensures that the relationship between future and practicing health professionals, and the communities in which they practice, will be better suited to unite around important health issues in a holistic way.

Rancho Cordova Children, Youth & Families Collaborative

The Cordova Community Collaborative is a group in the Rancho Cordova community of eastern Sacramento County.  CCC believes "all children should grow up in a family and community that provides for their health and well being. We are committed to strengthening families and linking them to community resources that enhance self-reliance and promote community involvement."  Folsom Cordova Unified School District's Family Support Services Director, Linda Burkholder, is CCC's CHPT Faculty.  Maura Reilly is the CHPT VISTA.

Harmony Health Family Resource Center
CHPT Faculty: Rachel Farrell, PA
CHPT VISTA: Jamie Ports

Harmony Health Family Resource Center assists families in the Linda community of Yuba County with referrals, in-home case management, counseling, parent workshops and support groups, women's support groups, youth groups and activities, and resource information. Director Rachel Farrell, PA, is the CHPT Faculty.


River Delta Healthy Start

River Delta Healthy Start serves the Courtland and Walnut Grove communities of rural south Sacramento county and offers support to rural and migrant farm worker families in the Sacramento River Delta area.  Healthy Start Coordinator Amy Chi is the CHPT Faculty.

River Oak - Oak Park Family Resource Center

River Oak Family Resource Center provides families in the Oak Park community with free, voluntary family support and home visitation services.  CHPT Faculty is Group Leader Katie Campbell.

Tahoe/Colonial Collaborative is a small, grassroots community organization situated in the
southeastern section of Sacramento, that works to connect families to each other and to resources for improved health, safety and overall wellbeing.  The CHPT Faculty is Coordinator Kelly Conley, and the CHPT VISTA is Sarah Kensky.

Valley Hi Youth & Adult Coalition
CHPT Faculty: Erika Green, MS, and Lisa Borrego

The Valley Hi Youth & Adult Coalition exists to create opportunities for Valley Hi children, youth and families to have successful experiences in school, community, personal positive growth and development. We value youth adult partnerships, fun and collaborative leadership. CHPT Faculty are Youth Programs Director Erika Green, and Coalition Coordinator Lisa Borrego.
